Job SummaryWe are seeking a dedicated Occupational Therapist to join a Skilled Nursing Facility on a 13-week contract assignment. The OT will work with residents to improve functional abilities, support rehabilitation goals, and promote independence through individualized treatment plans. This role offers a guaranteed weekly hour schedule and requires flexibility to work varying shifts.
Key Responsibilities- Conduct evaluations to assess residents’ functional status, mobility, and safety needs.
- Develop and implement patient-centered treatment plans based on clinical findings.
- Provide therapeutic interventions to support recovery, strength, mobility, and independence.
- Collaborate with an interdisciplinary care team to optimize patient outcomes.
- Document all assessments, progress notes, and care plans accurately and in a timely manner.
- Educate residents and caregivers on safe techniques, adaptive equipment, and ongoing home-care recommendations.
- Adhere to facility policies, professional standards, and state regulatory requirements.
- Active and current state Occupational Therapy license (required).
- Minimum of 1 year of experience in a rehabilitation or SNF setting.
- Strong communication, assessment, and clinical decision-making skills.
- Ability to work independently and adapt to varying shift schedules.
- Commitment to delivering high-quality, patient-focused care.
