Minimum Requirements

• Board Certified/Board Eligible: Board Certified or Board Eligible within 5 years of residency in Family Medicine or Internal Medicine; Hospice certification highly preferred;
• Certifications: BLS Required
• Licensure: Active OR license and OR DEA
