We re looking for a compassionate primary care physician to join a post-acute care team rounding in skilled nursing facilities across the Lancaster, Pennsylvania area. What You'll Do:Manage chronic conditions, diagnose illnesses, and develop care plans

Educate patients and caregivers on health goals and quality-of-life improvements
Foster a holistic approach to aging and recovery in a post-acute setting
What We're Looking For:At least1 year of primary care experience required
Experience in post-acute, but not required
Active and unrestricted Pennsylvania medical license and DEA
