-Experience: Minimum 1 year of experience in a primary care setting required.-Education: Graduate from an accredited Nurse Practitioner or Physician Assistant Program.-Licensure: Ability to obtain Nurse Practitioner (NP) license by the Oregon State Board of Nursing (OSBN) or Physician Assistant (PA-C) certified by the National Commission on Certification of Physician Assistants (NCCPA) and licensed by the Oregon Board of Medical Examiners (OBME) required. Must be eligible as a provider for Medicare, Medicaid, and other federal health programs.
