What You’ll Do:- Deliver patient-centered physical therapy services tailored to individual needs.
- Develop and implement personalized treatment plans to maximize physical function and recovery.
- Collaborate with a multidisciplinary team to ensure comprehensive care for residents.
- Track and document patient progress in compliance with industry standards.
- Educate patients and caregivers on exercises and strategies to maintain mobility.
What You’ll Bring:- Active Physical Therapist license in good standing.
- Graduation from an accredited Physical Therapy program.
- Strong interpersonal and communication skills.
- Passion for working in a skilled nursing facility and helping patients achieve their goals.
- Experience in geriatrics or a SNF setting (preferred but not required).
