Job Overview
We re seeking an Advanced Practice Provider to deliver the full scope of ambulatory primary care to geriatric participants within an integrated PACE model. The ideal candidate is compassionate, collaborative, and comfortable managing a broad range of clinical needs within their scope. Responsibilities include patient visits, telephone consultation, medical documentation, participation in quality initiatives, and coordination with multidisciplinary team members. Complex cases receive physician support as needed.
