The ideal candidate will have at least 2-3 years of primary care or urgent care experience and a background in community health, serving underserved patient populations.

Responsibilities and Duties- Provide home visits for an assigned patient panel
- Collaborate with the full care team to deliver comprehensive care
- Cover urgent care services as needed
Additional Information- Minimum 2-3 years of primary care or urgent care experience required
- Community health or underserved patient population experience preferred
- Acute care experience is a plus
- Must hold active OH license
- Required certifications: Basic Life Support (BLS), Drug Enforcement Administration (DEA)
- Family Nurse Practitioner Certification (FNP) required
