Provider s Role with the Company: Family Medicine Physician- Location: Seaford, DE (50mins from Dover)
- Outpatient primary care in the clinic setting
- Monday - Friday 8:30am - 5:30pm. 100% outpatient.
- Call: 1 in 6 weekend call. Video or Phone call only.
- Estimated Patient Volume: 25 per day
- Patient Age: 18+
- EMR: Cerner
- PA and NP support staff
- Assess, maintain, and improve the health status of individuals in the communities served through timely access to evidence-based acute, preventive, and chronic disease care.
- Serve as a role model of professionalism to all members of the interdisciplinary health care team. Collaborates with all other practitioners and clinical support staff to form a high performing team.
- Provide high-value care; demonstrate good stewardship of resources and consider risks, benefits, and patient preferences when making diagnostic or therapeutic intervention decisions.
- Documents all patient care activities and services provided in the electronic health record. Documentation must be clear, concise, accurate, updated, and in sufficient detail.

- Base Salary: 230K

Background Profile: Family Medicine Physician- Board Certified or Board Eligible Family / Internal Medicine
- Ability to hold an unrestricted state of Delaware MD or DO licensure
