American Traveler is seeking a Physical Therapist for a Home Health position requiring at least 1 year of recent experience, OASIS and HCHB proficiency, and flexibility to cover a wide territory.
Responsibilities- Home Health setting serving a largely geriatric patient population
- Daily caseload averages 5-7 patients, with a minimum productivity requirement of 6 visits per day or 30 points weekly
- Covers up to a 60-mile radius from the primary hub or office
- Day shift schedule, 5x8 hours, from 8:00 to 17:30
- On-call coverage required every 3rd to 4th weekend
- Productivity expectations and territory may vary, flexibility is required
- Uses Homecare Homebase (HCHB) EMR system
- First-time travelers are accepted
- Locals are accepted at the travel rate
- Physical therapists are expected to be flexible in covering the assigned territory
- On-call coverage is part of the regular rotation, typically every 3rd to 4th weekend
- Scrubs of any color are required; jeans are not permitted
- An iPad is provided for clinical documentation; cell phone is not provided
- Mileage is reimbursed at the current IRS standard rate
- Requested time off must be submitted at the time of application
- Former employees must have a minimum 6-month separation and HR approval to return
- Travel pairs are accepted but each must cover their own territory with separate transportation
- Current state PT license required if applicable
- Minimum of 1 year recent Home Health PT experience required
- OASIS experience required for Start of Care visits
- Proficiency in Homecare Homebase (HCHB) required
- Active BLS certification required
- Valid driver’s license and reliable personal transportation required
- Timely and accurate documentation and communication skills required
- Completion of Hospice/Home Health Checklist required for consideration
