The Opportunity
- Seeking Board Eligible/Certified Family Medicine physicians.
- Current CA state license is a plus.
The Practice
Adelanto Community Health Center – Adelanto, California
Ontario Health Center – Ontario, California
San Bernardino Health Center – San Bernardino, California
- As a Federally Qualified Health Center, this site is eligible for Public Student Loan Forgiveness (PSLF).
- Referral resources include WIC, Dental, Psych, Nutritionist, and a MediCal case worker.
- Facility hours for this location are Mon-Fri 8:30am-5pm.
- Patient mix is a majority adults.
- 15-18 patients per day per provider.
- Point of Care testing available at site; minimal labs.
