American Traveler is seeking a Physical Therapist for a home health assignment with at least 2 years of experience and an active license if applicable.
Responsibilities- Home health setting with a post-acute care focus
- Patient population includes orthopedic/joint replacement and geriatric patients with multiple comorbidities such as CHF and COPD
- Average caseload is 5 home visits per day
- PT staff perform start-of-care visits, especially with orthopedic patients when nursing is not ordered
- Uses Cerner charting system and Netsmart EMR; a laptop will be provided for EMR documentation
- Work schedule consists of 8-hour day shifts with occasional Saturdays for orthopedic patients
- No call or holiday shifts required
- Team includes an intake RN, manager, and nurse aides
- Main responsibilities include managing a caseload of home health patients and performing both routine and start-of-care visits
- Average daily census is 78 and is expected to increase with more therapy staff
- Scrub color is not specified
- Holiday coverage is not required
- No ventilator or call responsibilities
- Returning clinicians cannot have been employed by the related health system within the past year
- Orientation consists of 12 hours and is floor-specific

- Active state or compact (NLC) license required if applicable; pending licenses not accepted
- Current BLS certification required
- Ability to drive approximately 40 miles per day within the service area
- Must provide a copy of driver’s license for consideration
- Two professional references required, including at least one supervisor reference from the last 12 months and one peer or supervisor from the last 3 years which must state dates of employment and rehire eligibility
