Title: Physical Therapist (PT) – Skilled Nursing Facility (SNF)
Location: Murrieta, CA
Duration: 13 weeks (13–26 weeks)
Shift: Day shift | 5x8-hour shifts (08:00 – 16:30)
Weekly Work Guarantee: 36 hours
May include occasional weekend coverage and floating as needed
Job Responsibilities and Required and Preferred Skills & Requirements:
Responsibilities:
Provide physical therapy services to patients in a skilled nursing facility setting
Perform evaluations and develop individualized treatment plans focused on mobility, strength, balance, and functional independence
Deliver therapeutic interventions and monitor patient progress
Document evaluations, treatments, and outcomes in compliance with facility and regulatory standards
Collaborate with interdisciplinary healthcare team members to support patient-centered care
Required Qualifications:
Preferred Qualifications:
Minimum 1 year of experience in LTC and/or LTAC specialty
New graduates will be considered
Flexibility with assignment length and scheduling
