Job Overview:
We are seeking a highly skilled and compassionate Nurse Practitioner to join our dynamic team as a Long-Term Care Nurse Practitioner in a full time role. This position offers a unique opportunity to contribute to the well-being of our community's aging population by providing comprehensive healthcare services in long-term care settings. The ideal candidate will be an experienced and caring professional committed to delivering patient-centered care with a focus on quality of life.
Responsibilities:
Conduct comprehensive assessments of patients in long-term care facilities, including medical history, physical examinations, and medication reviews.
Develop and implement individualized care plans to address the unique needs of long-term care residents.
Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care.
Provide education and support to patients, families, and caregivers on disease management, preventive care, and overall health and wellness.
Monitor and manage chronic conditions, collaborating with physicians and specialists as needed.
Participate in regular team meetings and contribute to the development and improvement of patient care processes.
Stay abreast of current healthcare trends, regulations, and evidence-based practices related to long-term care.
Qualifications:
Master's or Doctorate degree in Nursing from an accredited program.
Current and unrestricted Nurse Practitioner license in the state of Maine.
Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
Strong interpersonal and communication skills with the ability to collaborate effectively with diverse healthcare professionals.
Dedication to providing compassionate and patient-centered care.
Knowledge of electronic health records(Epic) and proficiency in relevant medical software.
