Submission Requirements

ALL OF THE FOLLOWING SHOULD BE NOTED IN ADDITIONAL SUBMISSION DETAILS. REQUIRED PRIOR TO CLIENT PRESENTATION

• Board Certified or TRULY Board Eligible in Dermatology (note in additional submission details when is the expected graduation and when will the provider get their certification) REQUIRED no exemptions
• For Board Eligible - (within 3 years of Residency and to become BC within 1 year of employment) - (note in additional submission details_ - REQUIRED
• IA medical license (or ability to obtain) REQUIRED
• Willingness to build a new practice alongside a seasoned dermatologist- (note in additional submission details) REQUIRED

• Does the provider require visa support H1B? (We are open to H1B Visas only) (note in "Additional Submission Details" field) REQUIRED

• Clean malpractice history HIGHLY PREFERRED
