Care Manager LCSW at Providence Mission Hospital in Mission Viejo, CA. This position is Full-time and will work 8-hour, Day shifts.

Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. Required Qualifications:

• Education to meet certification, license or registration requirement.
• Upon hire: California Clinical Social Worker License
• 6 months of Clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting.

Preferred Qualifications:

• 3 years of experience in utilization management or case management.
• Managed care experience.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
