Registered Nurse Qualifications

• At least 12 months of recent Home Health RN experience (within the past 3 years)
• Proficiency with HCHB (Homecare Homebase) software – required
• OASIS documentation experience – required
• Active Wisconsin RN License or Compact RN License (must be current)
• Current BLS certification

Home Health Nurse Responsibilities

• Provide skilled nursing care to patients in their homes across Arbor Vitae, Minocqua, and Lac Du Flambeau
• Complete accurate and timely clinical documentation using HCHB and OASIS
• Collaborate with interdisciplinary healthcare professionals to optimize patient outcomes
• Educate patients and families on disease management, medications, and personalized care plans
• Travel between patient homes within the assigned Wisconsin territory
