The Clinical Pharmacist at BJC Home Care Infusion provides patient-centered care by overseeing medication therapy management, collaborating with healthcare teams, and ensuring safe and effective drug use. Responsibilities include supervising pharmacy personnel, managing medication preparation and dispensing, and participating in quality improvement initiatives. The role supports patients receiving home infusion therapy and promotes optimal outcomes through clinical expertise and education.

Position Details:
- Day Shift Hours: Mon – Fri (8:30am – 5pm)
- Call Rotation: Rotating On-Call with approximately 4 nights per month with paid call compensation — even if you're not called in!
- Saturday Rotation: Every 4-5 weeks, with premium pay for weekend work
- What You’ll Do:
- Provide patient-centered clinical care
- Collaborate with multidisciplinary teams at hospitals and physician practices to offer recommendations and ensure optimal patient outcomes
Requirements:
- Bachelor’s Degree or higher in Pharmacy
- 2 years or more of pharmacy experience
- Active Pharmacist License
- Current active pharmacist license in Missouri (RPh)
- Ability to obtain or currently hold Medication Therapy Services (MTS) certification
Experience:
- 5 years of hospital pharmacy experience (highly preferred)

Overview
BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.
BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.
The Home Infusion Therapy Department of BJC Home Care Services works closely with physicians, patients and caregivers to enable patients to remain or return home while receiving intravenous medications. Our services include 24 hour on-call pharmacy and nursing services, comprehensive patient education materials, patient assessment and drug administration monitoring, and coordination of professional services including home care nursing and laboratory services.

Role Purpose
Assess, monitor, initiate and modify medication use according to patient condition and age specific criteria. Ensure prompt and accurate medication preparation and dispensing. Prevent, direct and resolve medication-related issues. Supervise and direct supportive personnel. Counsel and educate to promote optimal drug therapy. Direct and assist in the procurement, storage, distribution and disposal of medications.
