Internal Medicine opening in New Hampshire - Located in Groveton, NH - Littleton 25m, Concord 90m - Full-time, permanent position - Seeking BC/BE - Rural Health ClinicOverview: - High elderly population - Experienced in Chronic Disease Management - 16-22 patients per day - 36 patient hours, 4hrs of admin time - May participate in day-time call rotation (no after hour call) - EMR is Meditech Expanse, Dragon dictation - Supported by an RN Team leader and medical assistant - Provide comprehensive medical care including physical exams, diagnoses, and treatment plans; develop personalized health plans for patients - Manage long-term treatment plans for patients with chronic illness, ensuring regular follow-ups and monitoring; manage acute care and chronic medical conditions and commodities. - Offer preventative care services such as immunizations, screenings (e.g., cancer, cholesterol, blood pressure), and well-ness check-ups.
