An innovative, national healthcare organization is seeking a Board-Certified Nephrologist to serve as a Lead Physician in a forward-thinking, value-based care model for patients with chronic kidney and end-stage renal disease.
This is an exciting opportunity to join a multidisciplinary team transforming the delivery of kidney care through data-driven, patient-centered, and home-based clinical services.
Position HighlightsFull-time, Monday Friday schedule
No nights, weekends, or call
Hybrid model: 60% remote (telehealth), 40% in-person home visits
Primarily clinical responsibilities
Variable daily volume (average 3 7 patients/day via telehealth)

Deliver comprehensive nephrology care for patients with chronic and end-stage kidney disease in home and telehealth settings.
Partner with interdisciplinary teams to develop and implement value-based initiatives focused on improving outcomes and reducing costs.
Utilize evidence-based practices and technology platforms to enhance care quality, data tracking, and performance outcomes.
Engage patients in education and shared decision-making to promote long-term kidney health.
Participate in multidisciplinary case reviews and continuous improvement initiatives.
MD or DO from an accredited institution
Board Certification in Internal Medicine and Nephrology
Active, unrestricted medical license and DEA registration
Strong clinical experience in nephrology across inpatient, outpatient, and dialysis settings
Excellent communication skills and collaborative leadership style
Experience or interest in value-based care and population health strongly preferred
