The Hospitalist Nurse Practitioner provides primary care within a skilled nursing facility, performing patient assessments, developing care plans, and coordinating treatment in collaboration with healthcare teams. This role offers flexible daytime hours, robust clinical support, and extensive professional development opportunities. Candidates must hold NP or PA licensure and demonstrate strong clinical and communication skills, with experience welcomed but not required.

As a TeamHealth post-acute care clinician, you will provide primary care coverage for patients in our skilled nursing facility Monday to Friday.
TeamHealth provides support for clinicians by creating a collaborative work environment through the cohesive GehriMed EMR system. In clinical situations, you have a responsive back-office team to offer additional support. Clinicians also have unlimited access to training materials and robust CME access through TeamHealth's employee portal.

Expected compensation is estimated salary range of $130,760 to $154,911 annually with no cap on productivity income potential. This full-time role is benefit eligible.

Key Responsibilities
- Perform comprehensive patient assessments, including medical histories and physical exams
- Develop, implement, and adjust individualized care plans based on patient needs
- Monitor progress and coordinate care to optimize outcomes
- Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
- Educate patients and families on health management and preventive strategies
- Maintain accurate, timely medical records in compliance with regulations
Qualifications
- Current NP or PA license (State of New York) and DEA
- Experience in post-acute, acute, emergency medicine, or clinic settings preferred but not required; new grads welcome to apply
- Strong clinical foundation and knowledge of healthcare regulatory standards
- Excellent communication and interpersonal skills
- Collaborative, team-oriented approach with a positive outlook
- Able to commute, relocate, and provide in person rounding in Amsterdam, NY
