Medication Management: Prescribe medications and formulate ongoing treatment and disease management plans. Qualifications Educational Background: Doctor of Medicine (MD), Doctor of Osteopathy (DO), or foreign equivalent. Residency Training: Completion of a three-year postgraduate medical training from a U.S. accredited residency program in Family Medicine or Internal Medicine. Experience: Open to both experienced and newly trained providers. Board Certification: Board Eligible/Board Certified in Family or Internal Medicine. Licensing: Must possess a current, unrestricted New York State license. DEA Registration: Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration. Medicare/Medicaid Enrollment: Must possess active Medicare/Medicaid Enrollment or be eligible to enroll. Life Support Certification: BLS or ACLS required. Communication Skills: Strong communication skills, both oral and written. Technology Proficiency: Comfortable with Electronic Medical Record (EMR) system and Microsoft Office.
