- Locum to Perm: This contract is designed for a provider seeking a long-term role.
- Flexible Schedule: Full-time hours with flexible start and end times to meet the FTE requirements. Coverage is needed every day except Thursday, including Saturdays (9 AM - 2 PM). Clinic hours run from 7 AM to 7:30 PM.
- Patient-Centered Practice: Join a Family Primary Care practice that sees patients from age 6 through end-of-life. The patient population is primarily adults over 45, with a manageable average daily census of 18 patients per day (PPD).
- EMR System: The facility uses Meditech for charting.
- Provide comprehensive primary care services to a patient population primarily consisting of adults and geriatrics.
- Manage a daily census of approximately 18 patients.
- Work collaboratively with the support staff to maintain the standards of a Patient-Centered Medical Home.
- Board Status: Board Certified (BC) or Board Eligible (BE) is required.
- Experience: Previous experience in a primary care setting is preferred.
- Licensure: An active Vermont (VT) State License is required. The facility is willing to license qualified candidates.
- Commitment: Must be available to provide physician help and access every day except Thursday.
- The target start date is January 5, 2026.
