Job Description
American Traveler is seeking a Physical Therapist with at least 2 years of experience for a home health assignment requiring BLS and OASIS experience.
Responsibilities
- Work in a home health post-acute care setting providing physical therapy services
- Patients are assigned by geographical region
- Typical diagnoses include orthopedic conditions, strokes, joint and spinal injuries
- Majority of hours are Monday through Friday, 8am-5pm with flexible shifts
- 8-hour day shifts with a weekend requirement every 3rd weekend
- Holiday coverage may be required for minor holidays, major holidays if needed
- Uses Cerner for EMR documentation
- Will be part of a multi-disciplinary team including PT, OT, Speech, with support from rehab aides and assistants
- Main responsibilities include assessing, planning, and providing physical therapy services in patients' homes
- Initial orientation specific to the assignment is provided and lasts 1.5 to 2 weeks
- Required to wear business attire and a white lab coat; jeans are not permitted
- May be assigned to cover multiple geographic regions within the service area
- Must not have been directly employed (full time, part time, or PRN) by CommonSpirit, CHI, or Dignity within the past year
- Open to experienced travelers; first-time travelers not specified
Requirements
- Active state or compact PT license is required for consideration; pending licenses will not be accepted
- License required if applicable
- Minimum 2 years of physical therapy experience
- BLS certification required
- Experience with OASIS documentation is required
- At least 1 year of experience working in PT/OT/Speech settings is required
- Home health experience is preferred but not required
- Copy of driver's license must be provided
- Two professional references required: one supervisor reference from within the last 12 months and one peer or supervisor reference from the last 3 years, including dates of employment and rehire eligibility
