Outstanding primary care group in the Northern California area is seeking an experienced nurse practitioner to join their growing team!
This position is a mainly clinic-based role, with a small portion of in-home duties within the local community. You will deliver high-touch care to geriatric patients with the goal of preventing hospital readmissions, as well as managing chronic conditions and coordinating care across specialties.
Ideal candidates will have experience working as a nurse practitioner in primary care, family medicine, internal medicine, gerontology, or another relevant field.
Description:
- Full-time permanent position - Primary Care
- Monday-Friday schedule
- Focus on geriatric high-risk patients
- See patients mostly in clinic (about 75%) and some in-home visits (about 25%)
- No telehealth
- Work alongside a robust support team of medical assistants, MD advisor, and clinical leadership
- Deliver high-touch care to geriatric patients with complex needs
- Prevent hospital readmissions and reduce avoidable utilization
- Conduct AWVs, risk assessments, medication management
- Manage chronic conditions and coordinate care across specialties
- Participate in IDT meetings and quality initiatives
Requirements:
- Experience as a nurse practitioner in primary care, internal medicine, gerontology, or other relevant fields
- California Nurse Practitioner license
- Spanish language skills (preferred)
