Our client is seeking a dedicated and experienced Family Nurse Practitioner to provide high-quality, patient-centered care in a community health setting. The ideal candidate will be passionate about serving diverse populations, promoting health equity, and working collaboratively within a multidisciplinary team.
Key Responsibilities:
- Provide comprehensive primary care services to individuals and families across the lifespan.
- Conduct health assessments, diagnose and treat acute and chronic conditions.
- Promote preventive care, health education, and wellness strategies.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Participate in quality improvement initiatives and community outreach programs.
Requirements:
- Current and unrestricted FNP license in New York.
- Minimum of 1 year of clinical experience preferred (new grads welcome to apply).
- Strong communication and interpersonal skills.
- Commitment to culturally competent care and community health principles.
