How you’ll make an impact in this role

  • Patient-Centered Care Management: Implement and monitor individualized care plans, ensuring every clinical intervention is tailored to the patient’s unique medical and personal needs.

  • Interdisciplinary Collaboration & Advocacy: Act as the primary liaison between patients, families, and the care team to facilitate seamless communication and advocate for high-quality, aligned care.

  • Clinical Assessment & Execution: Provide high-standard clinical support by accurately assessing health status, documenting changes, and executing physician orders to ensure the overall treatment strategy is met.

  • Continuity & Discharge Planning: Lead comprehensive discharge efforts to ensure a smooth transition from the facility, empowering patients and their families for the next stage of recovery.


What minimum requirements you’ll need

Licensure / Certification / Registration:

 

Required Credential(s):

  • Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire
