This position offers a manageable patient census, strong interdisciplinary support, and uncapped earning potential.Position OverviewThe physician will be responsible for delivering safe, effective, and cost-conscious care to patients in post-acute care communities. This role includes inpatient rounding across multiple care settings and close collaboration with interdisciplinary teams to ensure optimal outcomes and smooth care transitions.

• Patient volume: 15-20 patients per day
• Employment type: Full-time
• Compensation model: Fee-for-service (FFS)
• Estimated annual compensation: $252,000 - $302,000, with no cap on productivity income
• Benefits eligible full-time role

Key Responsibilities

• Provide high-quality, patient-centered care in post-acute care communities
• Perform inpatient rounding in skilled nursing facilities, inpatient rehabilitation facilities, and long-term care hospitals
• Collaborate with interdisciplinary clinical teams while adhering to CMS and Medicare guidelines
• Coordinate care transitions with hospice, palliative care, and home care agencies
• Ensure timely, accurate documentation and billing using the Gehrimed EMR system

Qualifications

• MD or DO
• Board certified in Internal Medicine or Family Medicine
• Active medical license in the state
• Strong clinical judgment and commitment to quality patient care
