Innovative Geriatric Medicine Leadership Role - Full-Time, M-F, No Weekends! We are seeking a dedicated and experienced Medical Director specializing in Geriatric Medicine to join a dynamic and mission-driven team in Auburn, Massachusetts. This full-time role supports the Program of All-Inclusive Care for the Elderly (PACE), an innovative model that delivers comprehensive, coordinated care to help seniors maintain independence within their communities. Position Overview Schedule: Full-time, Monday Friday Model: PACE (Program of All-Inclusive Care for the Elderly) Setting: Community-based care across multiple participant locations Key Responsibilities Conduct initial history and physical exams for new participants, along with scheduled reassessments. Develop personalized medical treatment plans and deliver ongoing clinical care across various care settings. Participate in the intake process, engaging with applicants and caregivers and supporting assessment decisions. Work closely with a multidisciplinary care team to review diagnoses, functional status, cognitive needs, and treatment options. Attend and contribute to team meetings, family meetings, and care planning discussions. Provide education to participants and families regarding health maintenance and chronic disease management. Perform comprehensive physical exams, order diagnostic tests, and document findings accurately. Participate in the on-call rotation as needed.
