About the Role:
Medadventures is seeking a dedicated and compassionate Occupational Therapist (OT) to join our team at a Skilled Nursing Facility (SNF). In this role, you will help residents improve their independence, safety, and quality of life through evidence-based occupational therapy interventions.
Key Responsibilities:
- Evaluate and assess residents’ functional abilities and therapy needs.
- Develop individualized treatment plans to improve mobility, ADLs (activities of daily living), and overall independence.
- Provide hands-on therapy sessions tailored to residents’ goals.
- Collaborate with an interdisciplinary team including physical therapists, speech-language pathologists, nurses, and physicians.
- Document progress notes, treatment plans, and discharge summaries in compliance with facility and regulatory requirements.
- Educate residents, families, and caregivers on therapy goals, safety, and home programs.
Qualifications:
- Current state licensure (OT) required.
- Graduate of an accredited Occupational Therapy program.
- Prior SNF or geriatric experience preferred, but new grads are welcome to apply.
- Strong communication and collaboration skills.
