Key Responsibilities
- Provide skilled physical therapy services to home health patients in the designated area
- Conduct patient assessments and develop individualized treatment plans
- Document patient progress using EPIC and Rover applications
- Travel within the local service area, with minimal drive time between visits
- Collaborate with healthcare team members to ensure comprehensive patient care
- Maintain compliance with all facility and regulatory requirements, including COVID vaccination and boosters
Required Qualifications
- Minimum of 1 year of skilled home health experience
- Valid and pending state license accepted
- BCLS certification required
- Reliable personal transportation
- Ability to work independently and manage a caseload effectively
Preferred Experience / Skills
- Experience with Oasis documentation
- Familiarity with home health care systems and protocols
- Ability to meet productivity targets (standard 30 points)
