We are looking for a nurse practitioner (NP) or physician assistant (PA) to join our dedicated post-acute care team in Manhattan, New York.
Position Details:
Full-time, 5 days per week, Monday through Friday
Patient volume averaging 17 patients per day
Post-acute care setting serving short-term and long-term patients
Key Responsibilities:
Provide primary care services including chronic disease management and illness diagnosis
Develop and implement individualized care plans
Order and interpret laboratory tests
Prescribe and monitor medications
Collaborate with physicians and coordinate care with healthcare teams
Educate patients and caregivers on managing chronic and age-related conditions
Build strong relationships with patients, families, and facility staff to ensure effective treatment delivery
Promote healthy lifestyle choices
Qualifications:
Minimum of 1 year of primary care experience as an NP or PA
Post-acute care experience preferred but not required
Active and unrestricted New York NP or PA license
Active and unrestricted DEA with prescriptive authority
