Education Requirements:

• Completion of four years of medical school.
• Successful completion of an American Board Medical Specialties approved residency program.

Licensure Requirements:

• Unrestricted Illinois MD/DO license.
• Illinois Controlled Substance License.
• Federal DEA.

Certification Requirements:

• Must be Board eligible upon hire.
• Board certified within the timeframe of the practitioner s primary specialty.
• Current CPR Certification (responsible for maintaining certification).
