The Navigator RN Per-diem Day serves as a nurse navigator collaborating with community organizations, faith-based groups, and educational institutions to promote comprehensive health and wellness. Responsibilities include health education, screenings, advocacy, case management, and tailored outreach to underserved populations. This role requires a California RN license, clinical experience in community health, and skills in volunteer management to support whole-person care in a hospital and community setting.
Description
The Nurse Navigator works with the manager and program staff to implement program goals and objectives. The Nurse Navigator collaborates with faith based communities, education institutions, community organizations and groups to promote the health and wellness of the whole person – body, mind, and spirit. He/she provides health education and health related services but not limited to classes, health screenings, health advocacy, health access support groups, case management and outreach to identified underserved populations. The nurse will tailor services to the unique health and wellness needs of the individual/family served.

Required Qualifications:
- Bachelor’s Degree in health related field, or 5 years clinical experience in community health settings.
- Upon hire: California Registered Nurse License
- Upon hire: National Provider BLS - American Heart Association
- 3 years healthcare experience.
- 1 year clinical experience in community health.
- Experience in working with and managing volunteers.
Preferred Qualifications:
- Coursework/Training: Training in Case management.
- Bachelor’s Degree in Nursing or Public Health Nursing.
- Experience in Public Health Nursing, Home Health Nursing, Community Health Setting, or in a Health Education setting.
- Experience as Faith Community Nurse.
- Experience in working with variety of community groups/organization, education institutions or faith based communities.
- Some supervisory experience.
- Expanded experience in working with volunteers from different cultures and with different levels of education.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
