Travel RN Job Details

• Position Type: Travel Registered Nurse – Home Health
• Location: Portage, Michigan 49002 (serving North/East Kalamazoo County; occasional coverage in Western Calhoun/Barry County)
• Schedule: Monday-Friday, 8:00am – 5:00pm
• Shift Duration: 8-hour day shifts
• Assignment Length: 13 weeks (Start: 10/13/2025, End: 1/11/2026)
• Hours per Week: 40
• Estimated Weekly Salary: $1920–$1970

Registered Nurse Qualifications

• Active Registered Nurse (RN) license in Michigan
• Minimum 1 year of recent Home Health RN experience
• Proficiency with HCHB (Homecare Homebase) and OASIS documentation systems
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities

Home Health RN Responsibilities

• Deliver skilled nursing care to patients in their homes throughout Portage and Kalamazoo County
• Conduct comprehensive patient assessments and create individualized care plans
• Document patient care using HCHB and OASIS systems
• Collaborate with interdisciplinary healthcare team members for optimal patient outcomes
• Educate patients and families on disease management, medication administration, and preventive care
• Travel within North/East Kalamazoo County and occasionally support Western Calhoun/Barry County
