Overview: We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to provide primary care services to residents in assisted and retirement living settings. The role includes diagnosing and managing acute and chronic conditions, prescribing medications, and promoting overall wellness. New graduates are welcome to apply!
Key Responsibilities:

• Conduct physical exams, diagnose, and manage medical conditions
  
• Prescribe medications and interpret diagnostic tests
  
• Provide education and preventive care guidance
  
• Collaborate with healthcare teams and refer to specialists as needed
  
• Manage chronic and acute illnesses in adult and geriatric populations
  

Requirements:

• Active NP or PA license (Required)
  
• 3+ years of EMR experience (Preferred)
  
• Background in Internal Medicine, Geriatrics, or Primary Care
