Job Requirements

• Education Requirements: Bachelor’s degree in Physical Therapy from an accredited institution
• Certifications Requirements: NBCOT certification, Montana PT license, BLS certification
• Experience Requirements: Minimum of 1 year of PT experience, preferably in SNF settings; experience with PDPM and Section GG is preferred but not mandatory
• Other Requirements: Ability to work Monday-Friday with flexibility for weekend shifts, strong documentation skills, excellent communication skills

Job Responsibilities

• Provide skilled physical therapy assessments and develop individualized treatment plans for patients in SNF settings.
• Implement and modify therapeutic interventions based on patient progress and evolving needs.
• Document patient progress and treatment outcomes accurately in NetHealth / PointClickCare systems.
• Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes.
• Educate patients and their families on exercises, treatment plans, and health maintenance strategies.
