Annual Salary: $140k-$160k (Varies depending on years of experience)
Our client is seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join their dynamic team at a Federally Qualified Health Center (FQHC) in Brooklyn. This is a full-time position serving a diverse patient population with a strong focus on community health, chronic disease management, preventive care, and health education.
Key Responsibilities:
- Provide comprehensive primary care services to patients of all ages, including physical exams, chronic disease management, acute care, and preventive screenings.
- Diagnose and manage common and complex medical conditions in accordance with evidence-based practice and FQHC standards.
- Collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, behavioral health specialists, and social workers.
- Participate in care coordination and quality improvement initiatives.
- Maintain accurate and timely electronic medical records (EMR) in compliance with organizational and regulatory requirements.
- Educate patients on wellness, prevention, and treatment plans.
- Support community outreach initiatives and contribute to the overall mission of improving access to high-quality healthcare.
Qualifications:
- Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of New York.
- Minimum of 2 years of direct clinical experience as a provider in a primary care or community health setting.
- Experience working with underserved and diverse populations preferred.
- BLS certification required; additional certifications (e.g., DEA) as needed for scope of practice.
- Excellent interpersonal and communication skills.
