Responsibilities:Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. Purpose-built to serve patients transitioning from hospital to home, Burnett features specialized rehabilitation spaces — dedicated gym spaces with the latest technology, a complete simulated home-like environment, and innovative therapy programs — all designed to help patients regain independence and quality of life in a post-acute setting.
- Perform physical therapy examinations, evaluations, and individualized treatment planning to restore mobility and improve physical function.
- Participate in interdisciplinary case conferences, discharge planning, and the coordination of care across the rehab team.
- Implement a plan of care for patients and evaluate the effectiveness of it in an ongoing, systematic process.
- Supervise physical therapist assistants, rehab therapy aides, and student clinical experiences to ensure quality care and professional development.
- Guide therapy and intervention through evidence-based research

Qualifications:Education and Experience
- Bachelor's Degree in Physical Therapy required
- 2 years of experience required
Licenses and Certifications
- PT- State of California Physical Therapist License required
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
