Job Description
American Traveler is seeking an experienced Home Health Physical Therapist for elderly patient care, requiring NJ licensure and BLS certification.
Responsibilities
- Home health setting providing care to elderly patients in Mercer County and North Burlington County
- This role is part of the PACE-LIFE (Living Independence for the Elderly) program
- Work schedule is Monday through Friday, 8:00 AM to 4:30 PM
- No on-call or weekend shifts required
- Regular home visits are required for patient care
- Responsibilities include conducting evaluations, developing and implementing individualized treatment plans, and providing therapy to improve mobility and function
- The role involves educating patients and caregivers on exercises and home-based care strategies
- You will collaborate closely with physicians, nurses, and case managers as part of a dedicated care team
- Maintaining accurate and timely documentation is an important component of this position
- No weekend or on-call work is required, supporting a work-life balance
Requirements
- Graduate of an accredited Physical Therapy program
- Valid NJ Physical Therapist license required if applicable
- AHA BLS certification required
- Minimum 2 years of recent experience with adult and geriatric populations
- Experience in home health and senior care is essential
- Strong communication, documentation, and problem-solving skills are required
- Must have reliable transportation
- Frequent standing, walking, and driving required
- Ability to occasionally lift up to 50 lbs., bend, and reach
- Experience in pelvic floor therapy preferred
- Training in Herman & Wallace Level 1A or equivalent preferred
