The Triage Hospice and Home Health Registered Nurse provides after-hours patient assessment and counseling for hospice and supportive care services in Missouri and Illinois. This role involves evaluating patient needs, determining the necessity of home visits, documenting care interactions, and identifying trends to inform staff education. The position requires an RN license in Missouri or Illinois, hospice experience, and consists of 12-hour shifts including some weekends and holidays.
Schedule
40 hours
Mostly days on-site in St. Louis
Some weekends and nights
Weekends can be remote or in office
3 holidays/year
12 hour shifts on weekends
Requirements
RN license in Missouri and Illinois
Must have at least one state and obtain the other state RN license within 3 to 6 months.
2 years of Hospice experience
Home Health experience is a plus!
Overview
