Clinic Locations: Bellwood, Berwyn, Brighton Park, Chinatown, Elgin, Garfield Ridge, and South Side

Compensation: $215K $245K Base Salary + Incentive Bonus

• Compensation: Base salary $215K $245K (based on experience and location)

• Bonus: Performance-based incentives tied to quality metrics
  

• Support Team: Scribe, Medical Assistant, and Social Worker

• Clinic Resources: Onsite lab with phlebotomist; RubiconMD specialist consultation platform

• EMR Systems: Canopy and Greenway

• Model: 100% outpatient, value-based healthcare

• Providers per Clinic: 3 5 (mix of MDs and NPs)

• Patient Experience: Complimentary van transportation within 5 miles; community wellness activities including yoga, exercise, dance, and social events
