- Monday, Thursday, and Friday 8 a.m. to 5 p.m. On-call 1-2 times weekend and every 4th-5th weekend.
- Coverage area for this position will be counties of Harrison, Grundy, Livingston, Davies, Caldwell, Mercer, and parts of Sullivan, Linn, Chariton, and Carroll.
- Ability to work one on one with patients
- Work autonomously as a RN
- Performs nursing assessments in the patients' home
- Utilizes full scope of nursing skills, including wound and ostomy care, PICC line care, chronic care education and medication reviews
- Coordinates patients' care - consulting other disciplines of the health care team
- Individualizes patient care plans and adjusting or modifying the nursing plan of care as appropriate
- Collaborates with the medical staff and the other disciplines to provide and enhance patient care
- Missouri and Kansas RN License Required
