Job Description
A Huntington Park based client is seeking a motivated and compassionate Nurse Practitioner to join a PACE team. An ideal candidate should practice excellent, evidence-based patient care, and should be comfortable treating older adults in a PACE facility. Managed care experience and community health experience are a plus.
Required Qualifications
- CA NP license
- CA DEA certificate before start date
- CPR certification
- Board certified in field
Responsibilities
- Assess and diagnose a variety of diseases and injuries in older adults
- Order diagnostic tests and laboratory studies, interpret results and create treatment plans based on results
- Perform therapeutic procedures with prioritizing preventative care
- Arrange referrals, consultations, and therapeutic services for patients on caseload
- Document all patient encounters efficiently and according to organization policy
