Key Responsibilities
- Conduct pre-enrollment UAS assessments for prospective participants using strong clinical judgment and nursing expertise
- Determine clinical eligibility and assess whether candidates meet criteria for PACE services
- Perform comprehensive clinical, functional, and environmental assessments across service areas
- Develop initial and prospective plans of care based on assessment findings
- Complete all required documentation, pre-screening forms, and assessment paperwork accurately and timely
- Conduct semi-annual reassessments as required
- Make plan-to-plan calls when indicated
- Evaluate appropriateness of therapy services for new participant referrals
- Participate in interdisciplinary and cross-functional meetings
- Maintain flexibility in scheduling and respond to changes in workload or urgent needs
- Travel throughout assigned service areas to complete assessments
- Active New York State Registered Nurse (RN) license
- Bachelor’s Degree in Nursing (BSN) required
- Prior experience conducting UAS assessments strongly preferred
- Strong assessment, documentation, and care coordination skills
- Ability to work independently while collaborating with interdisciplinary teams
