The Role:

• Full time, M-F
• Opportunity to pick up extra work for more pay if interested
• Nurse Practitioner will be working with patients to complete Risk Assessments in a Clinic and Home Health Setting
• Providers will be seeing 8 patients/day in a clinic setting or 4-5 patients/day for home health
• Provider will mostly be working in a clinic setting with the opportunity to see patients in their home when needed

Background Desired:

• Board Certified Nurse Practitioner
• Experienced with geriatric patients
• Bilingual in Spanish, required
