Requirements- Minimum Board Certification: Board Certified or Board Eligible (within 3 years of residency)
- License Requirement: CA license required (open to providers willing to obtain)
- CA Fluoroscopy Certification: Required or in progress
- Providers will also need to have or being working towards obtaining the CA FLURO
- The Following Certifications Are Required: ACLS
- Estimated Credentialing Timeframe: 90 days
- Fellowship: Required - Female Urology fellowship
- Will reimburse for licensing costs

- Robotic surgery (including partial nephrectomy, intraoperative ultrasound)
- Incontinence procedures
- Penile prosthesis
- Extracorporeal Shock Wave Lithotripsy (ESWL)
Additional Information- Must supervise Advanced Practice Providers
- No residents or teaching required
- Facility will accept one provider only
- CA licensure and fluoroscopy certification support available
