The Licensed Clinical Social Worker (LCSW) at Providence PACE provides comprehensive biopsychosocial assessments, treatment, and advocacy to elderly patients to support their independence and well-being. This role requires a Master’s degree in Social Work, Oregon clinical licensure, and the ability to perform community outreach including driving to patient locations. The position offers a hybrid work environment within a healthcare organization focused on serving vulnerable elderly populations through clinical and home-based care.
Description
The Licensed Clinical Social Worker (LCSW) independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems. The LCSW incorporates concepts and theories associated with life span development, neurobiology, and behavioral change.

Required Qualifications:
- Master's Degree - Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE).
- Oregon Clinical Social Worker License upon hire.
- Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
- 2 years - Relevant Social Work experience. May include CSWA experience.
Preferred Qualifications:
- 1 year - All EMPL's: Care Management: Experience in acute care settings.
- Social work experience.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
