Job Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health and OASIS experience.
Responsibilities
- Home health setting providing one-on-one physical therapy to patients in their homes
- Monday through Friday schedule with weekend rotation
- Role includes case management responsibilities
- Uses McKesson/Netsmart EMR, with experience strongly preferred
- Responsible for developing and implementing individualized care plans to maximize patient mobility and independence
- Collaboration required with interdisciplinary teams to ensure comprehensive patient care
- Utilization of a variety of therapeutic modalities and standard assessment techniques, including ROM, strength, and balance tests
- Scrub or business casual attire accepted for in-home patient care
- Potential for consideration of local candidates as no radius rule is indicated
Requirements
- Active Vermont PT license required if applicable
- Minimum 1 year of home health physical therapy experience required
- Demonstrated OASIS experience required for consideration
- Current ACLS and BLS/BCLS certifications required
- PALS and NRP certifications are listed as general certifications
- Rehabilitation-specific certifications such as ASHA, CHT, or ACP Training noted
- Strong ability to perform and document standard therapy measurement techniques
