Overview
The Nurse Practitioner (APRN) will provide high-quality primary care to adult and senior patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing individualized treatment plans in collaboration with a supervising physician. This is a safety-sensitive position involving direct patient care and medication administration.
Responsibilities
- Provide comprehensive care under physician supervision using sound medical judgment and ethical practice.
- Manage patients with acute and chronic illnesses, focusing on the older adult population.
- Maintain accountability for patient care and outcomes, consulting specialists when necessary.
- Coordinate with hospitalists when patients are hospitalized.
- Ensure compliance with regulatory standards (HIPAA, CLIA, state/federal regulations).
- Educate patients on prevention, risk reduction, and chronic disease management.
- Maintain accurate EMR documentation within 48 hours of each patient visit.
- Uphold patient confidentiality and adhere to all internal policies and procedures.
- Perform additional duties as assigned.
Qualifications
- Required: Bilingual (English/Spanish) proficiency
- Graduate of an accredited nursing program
- Certified and licensed Nurse Practitioner in the state of practice
- Minimum 1 year of nursing experience; 1 3 years as a practicing NP preferred
- Experience in primary care or geriatrics highly valued
- Proficiency in EMR systems and standard office software (Google Suite, Microsoft Office)
- ACLS/BLS certification required
- Strong communication, problem-solving, and organizational skills
Physical Requirements
- Ability to stand, walk, and assist patients for extended periods
- Lift/carry up to 15 lbs and operate medical equipment as needed
- Visual and hearing acuity required for patient care
- Must have reliable transportation
