A Locum Tenens Geriatrics Nurse Practitioner provides primary medical management and interdisciplinary care to elderly participants within the PACE program. Responsibilities include patient assessments, developing care plans, performing rounds in nursing homes, and educating patients and caregivers. The role requires collaboration with physicians, documentation using EMR systems, and on-call availability every third week.

Setting: PACE (Program of All-Inclusive Care for the Elderly)
Responsibilities:
- Provide primary medical management in collaboration with PACE Physicians
- Perform assessments and develop care plans for participants
- Evaluate and treat participant physical complaints
- Provide education to participants and caregivers
- Function as part of the Interdisciplinary Team (IDT)
- Provide oversight of the PACE Clinic
- Perform rounds in contracted nursing homes
- Document participant conditions and update medical records
On-Call/Weekend Requirements:
- On-call every third week (Monday Friday, 5:00 PM to 8:30 AM; Saturday and Sunday all day)
EMR System: Epic
- Active PA licenseRequired
- ANCC/AANP Board CertifiedRequired
- BLS CertificationRequired
- CDS Certification & DEA RegistrationRequired
- 5-7 years of NP experience, preferably in long-term care, with at least 1 year working with the frail/elderly populationRequired
- Clean/Clear malpractice history Highly Preferred
